START is a one-time comprehensive first visit designed to understand the full picture and create a clear plan forward.

During START, we focus on:

Your health history
Current symptoms and goals
An initial assessment and personalized care plan

If labs are recommended, they are billed separately and selected based on your individual needs.

No ongoing commitment is required to book your START visit.

Membership - CONNECT™

$150/Month

CONNECT is our membership option for patients who want ongoing guidance, follow-up, and clinical partnership over time.

CONNECT is designed for:

Continuity and progress
Follow-up support as your plan evolves
A steady relationship instead of one-off visits

If you start CONNECT at your first visit, your first month is free (and includes your initial follow-up).

Pay As You Go

$350/Hour

Prefer more flexibility? Pay As You Go lets you schedule follow-up care as needed.

This option is a good fit if you:

Want occasional check-ins
Prefer no monthly commitment
Are looking for episodic or consult-only care

Frequently asked questions

Do I have to join the membership?

No. You never have to join a membership to be seen at UpStream. Every patient starts with a comprehensive first visit. After that visit, you decide how you want to continue care. Some patients choose membership, others prefer to stay pay-as-you-go. Both options are always available.

What does the first month free include?

If you choose to start the membership at your first visit, the first month is free.That free month includes:

Your follow-up visit after the initial evaluation
Time to review your lab results and refine your plan
Ongoing clinical guidance during that first month

This allows you to experience how membership works before paying anything beyond the initial visit.

How do I cancel?

Membership is month-to-month.You can cancel at the end of any month by letting us know on or prior to the 25th of each month. There are:

No long-term contracts
No penalties
No pressure to stay

If membership stops being useful, you simply end it.

Are labs included?

No. Laboratory testing is not included in the visit fee or the membership.Labs are:

Personalized to your situation
Ordered only when clinically appropriate
Billed separately

We review recommended labs with you before ordering anything so you understand both the purpose and the cost.

Can I switch between options?

Yes. You can switch at any time.Patients often:

Start with membership, then move to pay-as-you-go
Begin pay-as-you-go and later choose membership

Your care doesn’t change — only how it’s structured. You remain in control of how and when you engage with UpStream. There is a waiting period of 3 months if rejoining membership from pay-as-you-go.
