Discontinuation of Activities and Data Destruction:

As of 3/31/2024, UpStream G.A.P.-Q terminated participation in the ACO REACH model in South Carolina, including the discontinuation of all marketing activities. In addition, UpStream G.A.P.-Q will archive REACH Beneficiary data it received and retain such data for the timeframes as required under applicable law.
